Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to Yellowbrick Learning, Inc, its university partners, affiliates, and agents (collectively "Yellowbrick"), to use my image, likeness, and/or any interview statements in any and all media now known or hereafter devised. This consent includes, but is not limited to:

(a) Permission to interview, film, photograph, tape, or otherwise make a video or audio reproduction of me; 

(b) Permission to use my name, title, and organization; and 

(c) Permission to use, edit, and distribute quotes from the interview(s), the film, photograph(s), or recordings of me, in part or in whole, for educational, promotional, or marketing purposes.

This consent is given in perpetuity, is irrevocable, and does not require prior approval by me. I hereby release Yellowbrick and its university partners from any and all claims arising out of the use of these materials, including but not limited to claims for defamation or invasion of privacy.

Any footage produced or derivative works created from such footage shall be considered the sole Intellectual Property of Yellowbrick and/or its applicable university partners.
